Qualifications

• Eighteen months of recent experience in an Acute Care Case Manager setting.
• Active RN License.
• BLS Certification.
• Degree from accredited nursing program.

Responsibilities

• Coordinate continuity of care for patients as liaison between family and healthcare organization.
• Work across HMOs, community health organizations, long-term care facilities, and other settings.
• Promote self-managed care and appropriate use of healthcare resources.
