Tidepoint Healthcare Staffing is hiring a Travel RN - Case Manager - Home Health

Shift: Days / 8a-5p, 8x5, 40 hrs per week

Length: 13 weeks

Requirements

• OR License
• 2+ Years Experience
• Previous clinical experience in an acute care setting, or home health setting assessing skilled care needs and equipment and health maintenance resource needs
• BLS
• BSN/MSN
• Graduate of an accredited school of medicine, social work, nursing or related medical occupation
• Previous experience with coordinating community resources for post-acute hospitalization care preferred
• Case Management Certification Experience preferred
• Previous Discharge Planning and/or Case Management experience preferred

Other Details

• CRASE training - required if travel assignment extends beyond a 6-month period
