Job summary

Unifi Healthcare is seeking an RN Case Manager to coordinate and deliver high-quality home-based care across the Knoxville area. The role focuses on developing, implementing, and evaluating personalized care plans, liaising with families, physicians, and the care team.

Qualifications

• Current RN licensure and valid nursing credentials.
• At least 3 years of progressively responsible clinical experience.
• Excellent writing, organizational, and time-management skills.
• Prior management or leadership experience.
• Ability to work independently, multi-task, prioritize, and collaborate in a team.
• Professional, courteous, compassionate, customer-oriented demeanor.
• Strong computer and internet skills; ability to use agency systems.
• Ability to lift 40–50 lbs and dress professionally for client visits.

Responsibilities

• Review client POC and submit weekly billing/documentation on time.
• Maintain accurate client information in QuickBase.
• Coordinate care with the team to ensure quality and satisfaction.
• Lead care plan development and oversee implementation per physician orders.
• Provide education to clients and families about benefits and services.
• Attend MD appointments and coordinate coverage as needed.
• Build rapport with families and manage caseload.
• Communicate field issues to DON/ADON and scheduling.
• Participate in monthly Case Management meetings and trainings.
