Job summary

TotalMed RN in Largo, FL is seeking a Travel Nurse RN Case Manager in Acute Care Case Management for a 13-week assignment. This role focuses on coordinating short- and long-term patient care, reducing hospital stays and readmissions through advocacy and collaboration with a multidisciplinary team.

The position requires BLS and at least 2 years of RN experience, with additional certifications possibly required. Travel, 40 hours per week, days shift, 8-hour shifts.

Qualifications

• BLS certification required.
• Minimum 2 years of RN experience.
• Additional certifications may be required before beginning an assignment.

Responsibilities

• Advocate for patients and coordinate short- and long-term care.
• Educate families about resources and health care options.
• Collaborate with RNs, doctors and social workers.
• Document and submit medical records.
• Communicate with Insurance to appeal decisions.
• Analyze results to improve financial outcomes and reduce readmissions.
