Job summary

TotalMed RN is seeking a Travel Nurse RN Case Manager for an Acute Care Case Management assignment in Largo, Florida. This 13-week travel position requires 40 hours per week, 8-hour day shifts, and offers travel benefits.

The RN will advocate for patients, develop care plans, educate families, and coordinate with a network of healthcare professionals to reduce readmissions and costs. The ideal candidate has 1–2 years of case management experience.

Qualifications

• 1-2 years of recent case management experience is required.
• RN Case Manager with active credentials preferred.
• BLS certification or higher is required.

Responsibilities

• Evaluate and assess new patients.
• Create and update comprehensive short- and long-term care plans.
• Educate families and patients about resources and health care options.
• Collaborate with RNs, doctors, and social workers.
• Document and submit medical records accurately.
• Communicate with Insurance companies regarding decisions.
• Analyze results and monitor variances to suggest actions to management.
