Responsibilities

• Day shift schedule, 8:00 AM–4:30 PM, 5x8 hours.
• Direct patient care with a designated visit productivity standard.
• Coordinate the interdisciplinary team to implement patient care plans.
• Manage visit utilization and control expenses for assigned caseload.
• Epic charting system experience preferred.
• Plan, organize, and direct all patient care services for assigned caseload.

Responsibilities

• Home health case management setting within a PACE program
• Day shift schedule, 5x8 hours, 8:00 AM to 4:30 PM
• Responsible for direct patient care with a designated visit productivity standard
• Coordinates the interdisciplinary team to implement patient care plans
• Manages visit utilization and controls expenses for assigned caseload
• Epic charting system experience preferred
• Plans, organizes, and directs all patient care services for assigned caseload in accordance with current standards and regulations
• May assist with agency-wide orientation, competency evaluations, and clinical consultation
• Accepts local candidates at the local rate
• Must reside within a 50-mile radius of the facility
• Max RTO of 7 days per contract, with only 1 holiday request approved per contract
• Holiday coverage includes New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day
• Returning staff must have a separation of at least 1 year to reapply as a traveler
• First-time travelers not considered for this position
