American Traveler, LLC. seeks an experienced RN Case Manager for a day shift at a teaching hospital in Massachusetts. Must hold an active MA RN license with 3+ years of acute care, plus BLS.

Responsibilities include coordinating care across the continuum, conducting admission/discharge assessments, and ensuring efficient resource use. The role requires strong communication, organizational, and negotiation skills, with collaboration across a multidisciplinary team.

Qualifications

• Active MA RN license required in hand; compact license not accepted.
• Current BLS certification (AHA or ARC).
• 3+ years clinical acute care experience in listed units.
• Experience in Rehab, Home Health, or LTC is considered.
• Ability to perform admission/discharge risk assessments.
• Knowledge of regulatory standards preferred.
• CCM/ACM certification preferred; RN-BC preferred.
• Meditech Expanse experience highly preferred.
• Knowledge of Medicare/Medicaid and third-party payer reimbursement.
• Knowledge of local community resources.
• Must have own vehicle.

• Case Management unit within a 298-bed teaching hospital.
• Manage healthcare needs and quality/cost outcomes across the continuum of care.
• Review patients at admission and concurrently throughout hospital stay.
• Perform transition care planning for post-hospital care arrangements.
• Day shift schedule, 7:00 AM-3:30 PM, five days per week.
• 10 or 12-hour shifts are not available.
• No weekend requirements outside of required holidays.
• No on-call requirements.
• Utilize a multidisciplinary team to coordinate services and resources.
• Participate in performance improvement initiatives.
• Prepare summary reports for management and regulatory agencies.
• Maintain confidentiality and build relationships with physicians.

American Traveler is hiring an experienced RN Case Manager for a day shift teaching hospital position requiring 3+ years of acute care experience and MA licensure.

• 3+ years of clinical experience in acute care settings such as internal medicine, surgery, orthopedics, mother/child, cardiology, or oncology
• 3+ years of clinical experience in Rehab, Home Health, or Long-Term Care settings will also be considered
• Ability to perform comprehensive admission and discharge risk assessments related to quality, financial, payer benefits, risk management, and patient satisfaction
• Knowledge of regulatory and accreditation standards (Joint Commission, DPH, HealthGrades) preferred
• CCM or ACM certification preferred
• RN-BC certification in Case Management preferred
• Meditech Expanse experience highly preferred
• Knowledge of Medicare, Medicaid, and third-party commercial payer reimbursement required

• Utilizes a multidisciplinary team approach to negotiate, procure, coordinate, and monitor patient services and resources
• Participates in performance improvement initiatives within the health system
• Requires strong organizational, analytical, and data comprehension skills to prepare summary reports for management, legal, or regulatory agencies as needed
• Requires excellent interpersonal, communication, collaboration, and negotiation skills with providers, patients, families, insurers, and government agencies
• Must be able to work independently and maintain confidentiality
• Must be able to establish and maintain relationships with physicians
• Holiday coverage required, including two full blocks: Thanksgiving Day/Friday and the following weekend, and Christmas Eve through New Year's Day including surrounding weekends
• Candidates must reside more than 50 miles from the facility to be eligible for this position
• RTO requests must be submitted at the time of profile upload; no more than 5 days of RTO will be accepted
