Qualifications

• Associate degree from an accredited PT school.
• Active license/permit to practice in state.
• Current CPR certification required.
• Valid driver license and ability to travel to locations.
• Ability to educate patients and families.

Responsibilities

• Provide patient care and document treatments per orders.
• Document interventions and set treatment priorities.
• Educate and counsel patients during treatment.
• Present patient status to supervising therapist.
• Deliver skilled treatment within agency and payer guidelines.
• Collaborate with team and attend meetings.
• Ensure timely daily notes and required forms.
• Meet productivity standards and participate in committees.
• Negotiate patient participation in plans.
