Job summary

Elara Caring is seeking a Home Health Registered Nurse (PRN) in the Boston area to deliver high-quality care in patients' homes. You will build meaningful patient relationships and provide personalized one-on-one care in a flexible, home-based setting.

Responsibilities include conducting comprehensive assessments, planning and evaluating care, and coordinating with the Interdisciplinary Team while ensuring HIPAA compliance.

Qualifications

• Current, unrestricted state RN license
• 1+ year experience in a clinical care setting as a nurse
• Able to travel 50% with a dependable vehicle and auto insurance
• Willing to travel within branch/office coverage area

Responsibilities

• Assess home health patients and families to identify physical, psychosocial, and environmental needs.
• Assume primary responsibility for caseload, including planning, coordinating, implementing, and evaluating the plan of treatment.
• Make initial evaluations and re-evaluate nursing needs during each visit; perform follow-up visits as needed.
• Communicate significant findings to the Clinical Supervisor, physician, facility, and other team members.
• Revise the plan of care in response to identified issues and notify the team.
• Maintain patient privacy and confidentiality per HIPAA rules.
