Job summary

Elara Caring is seeking a Part-Time Home Health Registered Nurse to provide one-on-one care in patients' homes across the Bellingham, MA area. This role offers a flexible schedule and a consistent, meaningful day with individual patients rather to a full-time clinical load.

Responsibilities include comprehensive nursing assessments, care planning, admissions, routine visits, and patient education, with mileage reimbursement during orientation.

Qualifications

• Current, unrestricted Massachusetts Registered Nurse license.
• At least one year of nursing experience in a clinical care setting.
• Home health or home care experience is highly preferred.
• Consistent availability for at least one full 8-hour day each week.
• Valid driver’s license, reliable transportation, and current automobile insurance.
• Ability and willingness to travel throughout the assigned territory.
• Ability to sit, stand, bend, lift, and move intermittently and lift 50-100 pounds.

Responsibilities

• Completing comprehensive nursing assessments.
• Developing and updating individualized plans of care.
• Performing admissions, routine visits, recertifications, and discharges.
• Monitoring patient progress and identifying changes in condition.
• Coordinating care with physicians, caregivers, and interdisciplinary team members.
• Educating patients and families about medications, disease management, and self-care.
• Helping patients remain safe and independent at home.
• Providing proactive interventions that help prevent avoidable hospitalizations.
• Completing accurate and timely clinical documentation.
