Elara Caring in Missouri seeks an experienced Registered Nurse – Home Health to deliver compassionate, high-quality care in patients' homes. You will handle planning, evaluating, and coordinating care for a full caseload while ensuring HIPAA compliance and strong communication with the care team.

The role requires an unrestricted RN license and 1+ year nursing experience, with willingness to travel up to 50%. Home health or hospice background is preferred, and a nursing degree is advantageous.

Qualifications

• Current, unrestricted RN license.
• 1+ year clinical nursing experience.
• Willingness to travel within assigned territory (up to 50%).
• Home Health, Hospice, or HomeCare HomeBase experience preferred.

Responsabilités

• Align care with Elara Caring mission and leadership direction.
• Maintain HIPAA confidentiality standards.
• Assess patient and family needs (physical, psychosocial, environmental).
• Manage full caseload including planning, coordinating, and evaluating care plans.
• Perform initial and ongoing patient evaluations and update care plans.
• Communicate changes in patient condition to clinical leadership and care team.
• Revise and coordinate care plans with interdisciplinary team.
