Qualifications

• Completion of an accredited physical therapy assistant program and current State licensure.
• Minimum of one year's therapeutic experience in a non-home health care environment; prefer two years.
• CPR certification; strong communication and organization skills.
• Minimum of twelve hours of continuing education annually.
• Must accept travel and varying working conditions.

Responsibilities

• Provides patient care services and direct treatment under the Physical Therapist.
• Updates the care plan with the therapist and physician, planning with patient/family.
• Plans monthly re-evaluations and weekly patient schedules with the therapist.
• Documents patient care data accurately and promptly showing progress.
• Coordinates with scheduling and care teams regarding referrals and visits.
• Participates in patient care conferences and mandatory in-services.
