Job summary

Liberty Health Whiteville, NC is seeking a Home Care Physical Therapy Assistant (PTA) to support patient recovery in Columbus/Bladen Counties. This full-time role works under a Physical Therapist, coordinating care plans and patient schedules to optimize outcomes.

The PTA must have an accredited program and state licensure, CPR, and 1+ year of relevant experience. Travel is required, and ongoing CE credits are expected.

Qualifications

• Completion of an accredited PTA program and current State licensure.
• Minimum of one year’s therapeutic experience in a non-home health care environment; two years preferred.
• Strong knowledge of physical therapy concepts and ability to treat patients with physician orders.
• CPR certified; strong communication and organizational skills.
• Minimum of twelve hours of continuing education annually.
• Willingness to travel and work as assigned.

Responsibilities

• Provides patient care services along with direct treatment and follow-up as assigned by the Physical Therapist.
• Updates the care plan in coordination with the therapist and physician, and plans with the patient/family toward adjustment.
• Plans monthly patient re-evaluations with the therapist and weekly patient schedule.
• Documents patient care data accurately, reflecting progress in the patient's condition.
• Submits the daily/weekly patient visit schedule to the Scheduler.
• Communicates with intercompany and intracompany contacts as necessary.
• Discusses treatment progress with the therapist, patient/family, and physician as needed.
• Coordinates with the Patient Care Coordinator for referrals to appropriate disciplines.
• Coordinates monthly supervisory visits from the therapist.
• Attends and participates in patient care conferences and mandatory in-services.
