Liberty Health in Lumberton, North Carolina, is seeking an experienced Home Care Physical Therapist (PT) for a full-time assignment. You will provide direct patient care, assess new patients with physician orders, and develop individualized plans of care to promote recovery and independence at home.

You will document progress, coordinate with the Patient Care Coordinator and other professionals, participate in education and orientation, and travel as needed.

Formación

• Bachelor’s degree in physical therapy from an accredited school.
• Current state licensure where therapy is provided.
• Minimum of one year’s experience in a non-home health care setting preferred.
• At least twelve hours of continuing education annually.
• Ability to travel and adapt to assigned working conditions.

Responsabilidades

• Provides patient care services with direct treatment and follow-up.
• Evaluates new patients with physician orders and develops plans of care.
• Documents patient care data accurately and timely, reflecting progress.
• Submits daily/weekly patient visit schedules to the Scheduler.
• Communicates with intercompany and intracompany contacts.
• Discusses reimbursement policies and treatment plans with patient/family and care team.
• Coordinates referrals to appropriate disciplines.
• Provides in-service education and supervision as needed.
• Assists with orientation of new employees and conducts research as required.
