Job summary

Qualifications

• Bachelor’s degree in a human services field; one year of professional experience in case management or related fields.
• Two years for Level II also acceptable with same degree.

Responsibilities

• Under the direction of a case management supervisor, provides services to individuals with intellectual and/or developmental disabilities living in their homes or community settings; conducts in-depth social histories and coordinates resources to meet consumer needs; serves as QIDP and conducts monthly monitoring.
• Must comply with Federal Home and Community Based Waiver regulations, state, and policy guidelines.
