Job summary

Oklahoma Department Of Human Services in Vinita, Oklahoma is seeking a Case Manager I/II to support individuals with intellectual and/or developmental disabilities in home and community settings. Two levels offer growth with education and experience requirements, including a required Case Manager Academy training.

Level I salary starts around $40,000.87 with full state employee benefits; Level II around $43,000.94 plus benefits. Travel with a valid driver's license is required.

Qualifications

• All New Employees will complete the Case Manager Academy Training Program.
• Level I requires a bachelor’s degree in a human services field and one year of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field; OR possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and one year of related experience.
• Level II requires a bachelor’s degree in a human services field and two years of professional experience working directly with individuals with intellectual and/or developmental disabilities or in social work, case management, special education, psychology, counseling, vocational rehabilitation, physical therapy, occupational therapy, speech therapy, nursing or a closely related field; OR possession of a valid permanent Oklahoma license as approved by the Oklahoma Board of Nursing to practice professional nursing and two years of related experience.

Responsibilities

• Under the direction of a case management supervisor, the position provides services to individuals with intellectual and/or developmental disabilities living in their own homes or community living alternatives; obtains in-depth social histories, develops and assures community and/or agency resources to meet the consumer's needs; serves as QIDP and provides monthly monitoring of the program.
• This position is required to meet compliance with Federal Home and Community Based Waiver regulations, state, and policy guidelines.
