Qualifications

• Bachelor’s degree in a human services field; includes related disciplines as specified in the posting.
• OR possession of a valid permanent Oklahoma nursing license to practice and one year working with individuals with IDD.
• For Case Manager job family, a human services degree includes most related fields except physical/natural sciences or mathematics.

Responsabilités

• Under supervision, provide services to individuals with IDD in home/community settings; obtain detailed histories and arrange resources to meet needs; act as QIDP and monitor programs monthly.
• Ensure compliance with Federal Home and Community Based Waiver regulations, state and policy guidelines.
