Responsibilities include providing outpatient physical therapy services under PT direction and using Cedaron EMR for documentation; the position offers a day shift with options of 4x10s or 5x8s and no weekends.

Candidates must reside 50 miles or more from the facility with a verifiable permanent address.

Qualifications

• Active Illinois PTA license required
• Minimum 2 years of recent outpatient experience
• 2 years of relevant setting experience
• Cerner EMR experience strongly preferred
• AHA certifications as required for the position
