Healthforce in Manheim, PA seeks a Physical Therapist for a 13-week travel assignment in a skilled nursing facility. The role starts on 11/02/2026 and includes guaranteed hours and a daytime schedule (08:00–16:30).

You will assess residents' PT needs, develop individualized plans, and deliver gait training, therapeutic exercises, and modalities. Collaborate with the interdisciplinary team to ensure safe transitions and quality care.

Qualificações

• Active Physical Therapist license in Pennsylvania (or eligibility to obtain licensure in PA).
• Doctor of Physical Therapy (DPT) degree from an accredited program.
• Current CPR/BLS certification.
• Prior experience in skilled nursing facility or geriatric rehabilitation preferred.
• Ability to work in a fast-paced SNF setting with strong documentation and communication skills.

Responsabilidades

• Assess residents' PT needs and develop individualized treatment plans to improve mobility, strength, balance, and functional independence.
• Deliver evidence-based physical therapy interventions, including gait training, therapeutic exercises, neuromuscular reeducation, and use of modalities as appropriate.
• Reassess progress regularly, adjust goals, and modify treatment plans to maximize outcomes.
• Document evaluations, progress notes, discharge plans, and communication with the interdisciplinary team in accordance with facility policies.
• Collaborate with nursing, OT, SLP, and medical staff to coordinate care and support safe transitions of care.
• Ensure patient safety during therapy, use proper body mechanics, and adhere to infection control and safety protocols.
