Job summary

HealthForce Healthcare is seeking a licensed Physical Therapist for a travel assignment in Lewistown, Missouri. This 13-week contract offers weekly pay ranging from $1,984 to $2,038, with a 5x8 day shift in a Skilled Nursing Facility, starting 10/26/2026.

Responsibilities include patient assessments, treatment planning, and supervising PTAs and therapy aides while collaborating with an interdisciplinary team to support resident function and discharge planning.

Qualifications

• Active Physical Therapy license in Missouri (state of practice).
• Graduate of an accredited Physical Therapy program.
• Experience in SNF or geriatric rehabilitation preferred.
• Strong clinical, communication, and organizational skills.
• Ability to work independently and as part of an interdisciplinary team.
• Willingness to supervise PTAs and therapy aides as allowed by licensure.

Responsibilities

• Conduct initial and ongoing assessments of residents’ mobility, balance, strength, gait, and functional limitations.
• Develop and implement person-centered treatment plans to restore or improve physical function, reduce pain, and promote independence.
• Deliver direct therapy interventions including therapeutic exercises, gait training, balance retraining, and the use of modalities as ordered.
• Monitor resident progress and adjust care plans; document treatment sessions, progress notes, and assist with timely billing.
• Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides in accordance with licensure requirements.
• Participate in interdisciplinary care team meetings and contribute to discharge planning; educate residents and families on home exercise programs and safety.
