Healthforce in Seaford, Delaware, is seeking a licensed Physical Therapist for a 13-week travel assignment at a skilled nursing facility. The role offers a guaranteed 35 hours per week with a consistent 8:00 a.m. – 4:30 p.m.

Qualifications

• Active physical therapist license or current eligibility per state regulations.
• Experience in a skilled nursing facility or geriatric rehabilitation setting preferred.
• Strong documentation and communication skills; ability to work within an interdisciplinary team.

Responsabilités

• Conduct initial PT evaluations and develop individualized treatment plans for residents
• Implement therapeutic interventions, including therapeutic exercises, gait training, balance work, and functional mobility activities
• Provide pain management techniques, ROM, strengthening, and modalities as appropriate to residents’ goals
• Monitor progress, document findings, and communicate with the interdisciplinary care team
• Reassess plans regularly and adjust treatments to maximize safety and outcomes
• Support discharge planning and facilitate transitions to activities of daily living and community re-entry when possible
