Job summary

Qualifications

• Valid Physical Therapy license in Connecticut (or eligible)
• Current BLS certification (AHA/ARC)
• Doctorate degree in Physical Therapy preferred

Responsibilities

• Evaluate patients’ physical conditions and develop personalized rehabilitation plans
• Implement therapy interventions to improve mobility and reduce pain
• Document evaluations, interventions and outcomes in the medical record
• Collaborate with physicians, nurses and other staff to optimize care
• Educate patients on home exercise programs and prevention strategies
• Maintain a safe, compliant treatment environment and assist with discharge planning
