Qualifications

• Active Physical Therapy - PT license in Indiana or compact state.

Responsibilities

• Provide physical therapy services to patients at a travel assignment in Fort Wayne, IN.
• Assess, plan and implement treatment programs per physician orders.
• Document patient progress and coordinate with facility staff.
