Qualifications

• Doctorate in Physical Therapy required or Bachelor's in Physical Therapy with NPTE eligibility.
• Graduate from an accredited program in Physical Therapy.
• Minimum 1 year of experience.
• Current state PT license or in process for licensure.

Responsibilities

• Assess referred patients and provide PT services.
• Develop and implement a treatment plan with goals.
• Document patient progress and communicate with physician.
• Collaborate with the care team to meet patient goals.
