Job summary

Fairview Hospital / Cleveland Clinic seeks a licensed Physical Therapist to assess patients, develop individualized rehabilitation plans, and deliver skilled interventions according to physician directives. You will document progress and adapt plans to patient needs, while educating families on therapy methods.

Required: DPT or BPT with NPTE, active state license or application, CPR certification, and at least one year of experience.

Qualifications

• Licensed or actively pursuing state licensure; CPR certification required.

Job description

Role: The Physical Therapist (PT) is responsible for assessing patients requiring rehabilitation services, developing personalized treatment plans, and providing skilled therapy interventions based on physician directives.

• Key Responsibilities: Conduct patient evaluations, establish and modify care plans, document patient progress, and educate patients and families on therapy methods. Collaborate with healthcare team members to achieve treatment goals.
• Qualifications: Must hold a Doctorate or Bachelor's degree in Physical Therapy with NPTE certification, graduated from an accredited institution, and possess at least one year of experience. Current state license or active application, along with CPR certification, are required. Additional requirements may be specified by the client facility.
• Skills: Strong assessment, treatment planning, documentation, patient education, and interdisciplinary collaboration skills. Ability to adapt therapy plans to patient needs and goals.
• Key Duties: Develops and implements individualized care plans, provides direct therapy services, evaluates patient progress, and adjusts interventions accordingly. Ensures documentation accuracy and educates patients and families about therapy techniques.
