Job summary

Northshore Home Health Care is seeking a Travel Physical Therapist (PT) to evaluate, diagnose, and treat patients with mobility impairments to improve function and quality of life.

You will develop personalized rehab plans, use manual and exercise therapies, monitor progress, document outcomes, and collaborate with medical teams; new grads welcome. The role requires license and BLS, willingness to travel, strong clinical and communication skills, and a commitment to patient safety.

• Responsibilities: Develop personalized rehabilitation plans, implement therapy techniques, operate equipment safely, monitor patient progress, document outcomes, collaborate with healthcare professionals, educate patients and caregivers, maintain a safe environment, engage in professional development, and assist with discharge planning.


• Qualifications & Skills: Valid Physical Therapy license, current BLS certification, strong clinical, communication, critical thinking, and interpersonal skills.
