Proforma Health is seeking a Travel Physical Therapist to evaluate, diagnose, and treat patients with physical impairments, aiming to improve mobility and reduce pain. You will create personalized, evidence-based plans and collaborate with healthcare professionals to ensure coordinated care.

Responsibilities include patient education, documentation, and adherence to safety standards while supporting ongoing professional development and discharge planning across multiple sites as needed.

Qualifications

• Valid Physical Therapy license per state regulations.
• Current BLS certification.

Responsibilities
