Apex Home Health, Hospice & Home Care is seeking a licensed Physical Therapist to evaluate, diagnose, and treat patients to improve mobility and reduce pain in a home health setting in Massachusetts. As a Physical Therapist, you will evaluate, diagnose, and treat patients to improve mobility, reduce pain, and enhance quality of life. Your duties include developing personalized rehabilitation plans, implementing therapy techniques, operating specialized equipment, assessing patient progress, and documenting outcomes. You will collaborate with healthcare professionals to ensure coordinated care, educate patients and caregivers on self-management, and maintain a safe treatment environment following regulatory standards. Additionally, you will participate in quality assurance and discharge planning activities, and perform other duties within your scope of practice.
Candidates must hold a valid Physical Therapy license and current BLS certification. Preferred qualifications include a Doctorate in Physical Therapy and relevant experience, though new graduates are welcome. Essential skills include critical thinking, effective communication, and proficiency in PC skills. Physical abilities required are lifting, moving equipment, and maintaining mobility. Interpersonal skills, technical dexterity, mental resilience, and sensory acuity are also necessary to succeed in this role.
