Job summary

Preferred Home Health Care & Nursing Services is seeking a licensed Physical Therapist to evaluate, diagnose, and treat patients to improve mobility and quality of life. You will develop personalized, evidence-based rehabilitation plans and collaborate with a multidisciplinary team to ensure coordinated care.

You will educate patients and caregivers on home exercises, monitor progress, document outcomes, and maintain safety and regulatory compliance while advancing your professional skills

Qualifications

• Candidate must hold a valid Physical Therapy license and current BLS certification.
• Preferred Doctorate degree in Physical Therapy; new graduates welcome.
• Strong critical thinking, communication, and safety protocol adherence required.

Role & Responsibilities

The Physical Therapist evaluates, diagnoses, and treats patients to improve mobility, reduce pain, and enhance quality of life. They develop personalized, evidence-based rehabilitation plans using exercise, manual therapy, and specialized equipment. The role involves assessing patient progress, adjusting treatment strategies, documenting outcomes, and collaborating with healthcare professionals to ensure coordinated care. They also educate patients and caregivers on home exercises, injury prevention, and self-management techniques, while maintaining a safe, compliant environment and participating in quality improvement activities.

Qualifications & Skills

Candidates must hold a valid Physical Therapy license and current BLS certification. Preferred qualifications include a doctorate degree in physical therapy and relevant experience, though new graduates are welcome. Essential skills include critical thinking, effective communication, proficiency in English, technical motor skills, physical endurance, interpersonal effectiveness, and the ability to work under pressure. Sensory acuity and adherence to safety protocols are also required.
