Proforma Health is seeking an Outpatient Physical Therapist (PT OP) to provide therapy services in an outpatient setting. You will deliver high-quality physical therapy care, ensure proper documentation, and comply with facility and regulatory standards, while working day shifts with a minimum of 40 hours weekly.

Valid active license, at least 13 years of experience, and adherence to state licensure requirements are expected. Equipment will be provided.
