Preferred Home Health Care & Nursing Services is seeking a Physical Therapist for a travel assignment. You will evaluate, diagnose, and treat patients to improve mobility and reduce pain, develop personalized rehab plans, and document progress to support outcomes.

Qualifications

• Valid Physical Therapy license per state regulations.
• Current BLS certification.
• Doctorate in Physical Therapy preferred.
• Additional certifications welcome.

Responsibilities

• Evaluate patients' physical conditions and identify treatment needs.
• Create and implement evidence-based rehab programs.
• Operate therapy equipment safely and effectively.
• Assess progress and adjust treatment plans accordingly.
• Document evaluations, interventions, and outcomes.
• Coordinate care with healthcare team members.

• Maintain infection control and safety protocols.
• Engage in ongoing professional development and quality initiatives.
• Assist in discharge planning and community reintegration.
