Preferred Home Health Care & Nursing Services is seeking a Travel Physical Therapist to evaluate, diagnose, and treat patients in home settings, aiming to improve mobility, reduce pain, and promote independence. The role covers in-home rehabilitation and education for patients and caregivers.

The therapist will collaborate with healthcare teams, monitor progress, and ensure safety and infection-control standards while providing flexible scheduling to support diverse patient needs.

Qualifications

• Valid physical therapy license per state regulations.
• Current BLS certification.
• Strong clinical, communication, and interpersonal skills.
• Proficiency in PC skills and ability to follow instructions.
• Physical ability to move equipment and assist patients.
• Effective communication with diverse healthcare teams.
• Ability to work under pressure and adapt to irregular schedules.

Responsibilities

• Assess patients' physical conditions using tests, examinations, and history to identify treatment needs.
• Develop and implement personalized rehabilitation plans utilizing exercise, manual therapy, and specialized equipment.
• Monitor patient progress, adjust treatments, and document outcomes accurately.
• Collaborate with healthcare professionals to ensure coordinated care.
• Educate patients and caregivers on home exercises, injury prevention, and self-management.
• Maintain a safe environment adhering to infection control and safety standards.
• Participate in quality assurance, professional development, and discharge planning activities.
