Qualifications

• 1+ year of PT experience in skilled nursing or post-acute care settings.

• Current authorization to work in the United States.

Responsibilities

• Conduct PT evaluations and develop plans of care for residents in SNF.
• Implement therapeutic exercises, gait, balance, ROM and mobility activities.
• Reassess progress, modify treatment plans and document outcomes.
• Coordinate with interdisciplinary team; participate in discharge planning.
• Provide patient and caregiver education on exercises, safety, fall prevention, and home programs.
• Maintain documentation in EMR and comply with facility policies and infection control standards.

• Reassess patient progress, modify treatment plans, document outcomes, and communicate with the interdisciplinary team.
• Provide patient and caregiver education on exercises, safety, fall prevention, and home programs.
• Maintain accurate documentation in the electronic medical record and comply with facility policies and infection control standards.
• Coordinate with therapy assistants and other disciplines; participate in discharge planning and transitions.

Requirements

• Active Physical Therapist license in California or eligibility to obtain licensure.
• 1+ year of physical therapy experience in skilled nursing or post-acute care settings.
• Ability to work in a skilled nursing facility environment and commit to a travel assignment.
