Job summary

Healthforce is seeking a Physical Therapist for a Skilled Nursing Facility travel assignment in Riverside, California. The role starts 11/16/2026 for a 13-week duration, with 8.5-hour days from 08:00 to 16:30, five days a week.

The ideal candidate holds a California PT license (or eligibility), with at least 1 year in SNF or post-acute care, strong documentation and teamwork, and current US work authorization. Dedicated travel support and onboarding guidance are provided.

Qualifications

Active Physical Therapist license in California or eligibility to obtain licensure.
1+ year of PT experience in skilled nursing or post-acute care settings.
Willingness to travel for a travel assignment.
Strong documentation and communication skills; team-oriented and reliable.
Graduation from an accredited PT program (DPT or equivalent).
Current authorization to work in the United States.

Responsibilities

Conduct PT evaluations and develop plans of care for residents in SNF.
Implement therapeutic exercises, gait, balance, ROM and mobility activities.
Reassess progress, modify treatment plans and document outcomes.
Coordinate with interdisciplinary team; participate in discharge planning.
Provide patient and caregiver education on exercises, safety, fall prevention, and home programs.
Maintain documentation in EMR and comply with facility policies and infection control standards.
