Job summary

Proforma Health Inc is seeking a Travel Physical Therapist to provide skilled services in a Skilled Nursing Facility. You will assess patient needs, develop and implement treatment plans, and deliver therapy in a patient-focused setting.

The role requires a valid PT license (including compact), BLS certification, and at least one professional reference, with travel between Cedar Rapids and Keystone. Competitive pay and comprehensive benefits are included.

Key Duties

Work 8 hours per day, 40 hours weekly, in a supportive team setting. Flexibility to float between Cedar Rapids and Keystone locations. Maintain documentation, collaborate with staff, and contribute to patient recovery.
