SnapCare is seeking a licensed Physical Therapist for a 13-week travel assignment in Fort Wayne, IN. Multiple start dates are available: week of Oct 26, week of Nov 23, and week of Dec 7.

Typical schedule is Monday through Friday, 8:00 AM to 5:00 PM with occasional weekend rotations. You will evaluate patients, develop and adjust treatment plans, supervise therapy services, educate families, and coordinate with an interdisciplinary team to overcome discharge barriers.

Competenze

• Minimum of 2 years of physical therapy experience required.
• LTACH experience highly desired.
• BLS certification required prior to first shift; Indiana state license required.

Mansioni

• Evaluate assigned patients and develop, implement, and modify treatment plans per physician direction.
• Delegate and direct discipline-specific therapy services for assigned patients.
• Design functional treatment programs to support return to community activities.
• Serve as an educator for patients, families, and coworkers.
• Collaborate with interdisciplinary team to address discharge barriers including equipment trialing and home exercise program development.
• Complete medical records clearly and timely, including daily notes, goals, functional quality indicator assessments, and discharge documentation.
• Manage individual, concurrent, and group treatment sessions based on patient care needs.
• Ensure care follows signed physician orders and insurance approvals.
• Participate in student programs, supervising and evaluating students.
• Adhere to state practice acts and all regulatory requirements.
