Job summary

American Traveler is seeking a Physical Therapist for a Long Term Care facility. The role requires 2+ years of PT experience and a TX or compact license where applicable. The position features a day shift with 8-hour shifts and no weekend or on-call requirements.

You will perform patient evaluations, establish care plans, oversee discharge planning, and document thoroughly per policy. Collaboration with staff and ongoing professional development are emphasized.

Qualifications

• Minimum 2 years of PT experience required.
• TX or compact license required if applicable.

Responsibilities

• Long Term Care (LTC) setting within the Physical Therapy unit.
• Day shift schedule, 8-hour shifts.
• No weekend requirements.
• No on-call requirements.
• Performs patient evaluations including history, systems review, and administration of tests and measures.
• Establishes individualized plans of care and selects treatment techniques addressing identified impairments per physician orders.
• Conducts ongoing reassessment of patient response to treatment.
• Initiates and manages discharge planning from the first visit through treatment completion.
• Supervises and delegates to ancillary staff and students per state law and department policy.
• Maintains accurate, timely documentation in accordance with department policy and accreditation requirements.
• Communicates with patients, families, and healthcare team members using AIDET principles.
• Assesses and addresses educational needs of patients, families, and caregivers.
• Participates in ongoing professional development and quality improvement activities.
• Maintains a safe environment for patients, visitors, and staff in compliance with safety standards.
• Functions within scope of practice as defined by state practice act.
