Job summary

Jackson Therapy Partners is seeking a travel Home Health Physical Therapist for a travel assignment in Hickory, North Carolina. The role is a 18-week engagement with 40 hours per week and 8-hour shifts, delivering in-home physical therapy services to patients recovering from injuries and managing chronic conditions.

Ideal candidates hold a PT degree and an active NC license with BLS/CPR certification. Local travel between patient homes is required, with housing and license reimbursement

Qualifications

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by APTA.
• Active North Carolina Physical Therapist license required to start the assignment.
• Current BLS/CPR certification from AHA or ARC required.
• Experience in home health settings preferred; case management of PT patients and home safety assessments.
• Ability to manage a full caseload, complete timely documentation, and coordinate care with nursing and other disciplines.
• Valid driver’s license and reliable transportation for patient visits. Mileage reimbursement provided between visits.
• Minimum education requirement: Bachelor’s Degree.

Responsibilities

• Provide in-home evaluations and therapeutic interventions.
• Discharge planning and case management in coordination with physicians and interdisciplinary team members.
• Maintain documentation accuracy to support quality outcomes and regulatory compliance.
• Collaborate with nursing and other disciplines to support safe discharges and functional outcomes.
• Local travel between patient homes within assigned coverage areas.
