Experience with equipment such as Hoyer lifts, walkers, slings, and sliding boards preferred
Strong time management and documentation skills required

Responsibilities

Home health setting covering Dayton, Brookville, Eaton, and West Alexandria
Adult and geriatric patient population
Full-time schedule, 8:00–17:00, Monday–Friday, no weekends (flexible weekend visits possible)
Weekly productivity: 7 visits per day / 35 per week
Uses Home Care Home Base EMR system
Training provided for candidates without prior Home Care Home Base experience
Duties include patient assessment, home exercise program development, and patient/family education
Focus on safety, injury prevention, and identifying environmental barriers in the home setting
Local candidates within 50-mile radius encouraged to apply
Contract allows up to 14 days off; longer contracts available through March end
