Job summary

Cayuga Health in Ithaca, NY is seeking a Physical Therapist to provide services across our agency network under the Director of Rehabilitation Services. The position requires travel to patient sites (100%).

You will evaluate patients, develop treatment plans, and document care per physician orders, while collaborating with the clinical team. A NYS PT license, 2+ years of experience, and a valid NYS driver’s license are required.

Qualifications

• Licensed and registered Physical Therapist in NYS.
• Minimum 2 years supervised clinical experience.
• Graduate of an accredited physical therapy program.
• Valid NYS driver’s license with reliable transportation.

Responsibilities

• Evaluates functional abilities, develops individualized treatment plans, and provides physical therapy interventions.
• Completes timely evaluations, re-evaluations, progress notes, discharge summaries, and OASIS assessments.
• Collaborates with physicians and the clinical team, assesses the need for additional services, and contributes to the overall plan of care.
• Educates patients and families on therapy and participates in staff instruction, orientation, and supervision.
• Assists with development of physical therapy policies and procedures, communicates progress with physicians.
