Responsibilities

• Typical caseload: 5-6 units per day with schedule metrics
• Day shift: 8:00am–4:30pm, Monday–Friday
• Self-scheduling with area-based visits; no weekends/holidays
• Uses Epic EMR for documentation; works with a team of 4 PTAs
• Provide home health PT across a multi-county territory; adjust coverage by candidate location
• Orientation at Gassaway office focused on Epic docs

Responsibilities

• Home health setting covering an 8-county service area including Braxton, Gilmer, Calhoun, Nicholas, Webster, Clay, Randolph, and Pocahontas counties
• Patient population includes general orthopedic conditions (hips, knees), generalized weakness, strengthening needs, and cardiac rehab
• Typical caseload of 5-6 units per day (SOC=2, Discharge=1, Recert=1.5, Regular visit=1)
• Day shift schedule, 8:00am-4:30pm, Monday-Friday
• Self-scheduling allowed, with flexibility to arrange patient visits by area
• No weekend or holiday requirements
• Uses Epic EMR for documentation
• Works alongside a team of 4 PTAs
• Main responsibilities include providing home health physical therapy services across an assigned multi-county territory
• Preference for candidates able to help cover Webster/Nicholas counties, though coverage area may be adjusted based on candidate location
• Department-specific orientation takes place at the Gassaway office, focused on Epic documentation and tailored to clinician needs
• Dress code includes navy scrubs or khaki bottoms with a navy polo
• Cancellation policy allows for 2 shifts per 13-week assignment
