Job summary

Apex Home Health, Hospice & Home Care seeks a Travel Home Health Physical Therapist to provide outpatient physical therapy services in patients' homes across a multi-county area in West Virginia. You will handle orthopedic, weakness, and cardiac rehab cases while managing a 5-6 unit daily caseload and documenting in Epic EMR.

Requirements include a minimum of 2 years of PT experience, BLS, WV licensure if applicable, and a proactive approach to Epic documentation.

Role:

Travel Home Health Physical Therapist responsible for providing outpatient physical therapy services in a home health setting across a multi-county area.

Responsibilities:

Deliver therapy for patients with orthopedic, weakness, and cardiac rehab needs; manage a caseload of 5-6 units daily; utilize Epic EMR; collaborate with a team of PTAs; ensure patient care quality and documentation accuracy.

Qualifications & Skills:

Minimum of 2 years of physical therapy experience, BLS certification, WV licensure if applicable, positive attitude towards learning Epic documentation, flexibility in coverage area, and ability to work independently with self-scheduling.

Key Duties:

Administer therapy during day shifts (8am-4:30pm, Monday-Friday); provide services in patients' homes; support general orthopedic and cardiac rehab cases; participate in department orientation; adhere to dress code and cancellation policies.
