Job summary

Healthforce seeks a Physical Therapist – General for a 14-week travel assignment in Urbandale, IA (home healthcare). Start 11/16/2026; 40 hours per week; 8-hour shifts. Responsibilities include patient assessments, Plan of Care development, and treatment planning for neuromuscular, neurological, and orthopedic conditions.

Requirements: active PT license, CPR, valid driver’s license, 1–2 years in PT (acute or rehab); home health preferred. Travel and credentialing support provided.

Qualifications

• Current/state license as a Physical Therapist – Required
• CPR certification – Required
• Current driver’s license with reliable transportation and insurance – Required
• 1–2 years of experience as a Physical Therapist in an acute care or rehabilitation setting – Required
• Previous home health/home care experience – Preferred

Responsibilities

• Perform clinical assessments to identify patient needs and appropriateness of physical therapy
• Develop, implement, and review a Plan of Care (POC); document progress notes and other clinical records; revise the POC as needed
• Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on referrals and evaluations
• Instruct patients and families on transfer techniques and safe use of equipment; arrange appropriate rehabilitative equipment as needed
• Educate patients and families about therapeutic procedures, considering age, condition, culture, and learning readiness
• Collaborate with an interdisciplinary team and supervise other staff when applicable; apply infection control and universal precautions
