Protouch Staffing is seeking a travel Physical Therapist for a 13-week assignment in Urbandale, Iowa. The practitioner will deliver home health physical therapy services, work M-F 8am-5pm, and cover 40 hours weekly with on-call potential as patient needs arise.

Requirements include an active Physical Therapist license, CPR, driver's license with reliable transportation, and 1-2 years of acute care or rehabilitation experience. Home health experience and HCHB EMR familiarity are preferred.

Qualifications

• 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
• Previous home care experience - Preferred

Responsibilities

• Provide Physical Therapy services in home health/travel setting.
• Collaborate with care team to develop and adjust patient plans.
• Document patient progress and outcomes in HH EMR (HCHB/OASIS).
• Adhere to HH guidelines and complete required on-call duties as needed.
