HealthForce Healthcare in Silverdale, WA is seeking a Physical Therapist for a 14-week home healthcare travel assignment starting 10/26/2026. The position offers a guaranteed 40 hours per week and a weekly pay range of $2,765–$2,808, with 8-hour shifts.

Responsibilities include clinical assessments, plans of care, neuromuscular/neurological/orthopedic therapy, patient education, and coordination with a multidisciplinary team.

Formación

• Active PT license in Washington or eligibility.
• CPR certification required.
• 1–2 years PT experience in acute care or rehab settings.
• Bachelor of Science in Physical Therapy from an APTA-approved program.

Responsabilidades

• Conduct clinical assessments to identify patient needs and determine appropriateness of physical therapy
• Develop, implement, and revise a Plan of Care with progress notes and documentation; collaborate with physicians and an interdisciplinary team
• Plan and deliver therapy for neuromuscular, neurological, and orthopedic conditions based on medical referral and evaluative data
• Instruct patients and families in safe transfer techniques and the use of equipment; select and adapt equipment for home environments
• Provide patient and family education tailored to age, condition, culture, and learning readiness; promote infection control and universal precautions
• Coordinate with and, when applicable, supervise other team members to ensure care aligns with patient needs and preferences

Responsibilities

• Conduct clinical assessments to identify patient needs and determine appropriateness of physical therapy
• Develop, implement, and periodically revise a Plan of Care (POC) with progress notes and related documentation; collaborate with physicians and an interdisciplinary team
• Plan and deliver therapy for neuromuscular, neurological, and orthopedic conditions based on medical referral and evaluative data
• Instruct patients and families in safe transfer techniques and the use of equipment (walkers, wheelchairs, canes, braces, prosthetic devices); select and adapt equipment as needed for home environments
• Provide patient and family education tailored to age, condition, culture, and learning readiness; promote infection control and universal precautions
• Coordinate with and, when applicable, supervise other team members to ensure care aligns with patient needs and preferences

Requirements

• Active Physical Therapist license in the state of Washington (or eligibility as required) – Required
• Current CPR certification – Required
• Valid driver’s license with reliable transportation and adequate auto insurance – Required
• 1–2 years of PT experience in acute care or rehabilitation settings – Required
• Bachelor of Science in Physical Therapy from an American Physical Therapy Association–approved program – Required
• Ability to work in a home healthcare setting and travel locally as needed
