Responsibilities include assessing patients, creating and updating a Plan of Care, and delivering PT services for neuromuscular, neurological, and orthopedic conditions. Travel locally may be required; CPR and WA licensure are mandatory.

Qualifications

• Active Physical Therapist license in WA, or eligibility.
• Current CPR certification is required.
• Valid driver's license with reliable transportation.
• 1–2 years PT experience in acute care or rehab settings.

Responsibilities

• Conduct clinical assessments to identify patient needs and determine appropriateness of physical therapy.
• Develop and revise a Plan of Care with progress notes; liaise with physicians and team.
• Plan and deliver therapy for neuromuscular, neurological, and orthopedic conditions as per referrals.
• Instruct patients in safe transfer techniques and equipment use for home environments.
• Educate patients and families, promote infection control and universal precautions.
• Coordinate with and supervise team members to align care with patient needs.

Responsibilities

• Conduct clinical assessments to identify patient needs and determine appropriateness of physical therapy.
• Develop, implement, and periodically revise a Plan of Care (POC) with progress notes and related documentation; collaborate with physicians and an interdisciplinary team.
• Plan and deliver therapy for neuromuscular, neurological, and orthopedic conditions based on medical referral and evaluative data.
• Instruct patients and families in safe transfer techniques and the use of equipment (walkers, wheelchairs, canes, braces, prosthetic devices); select and adapt equipment as needed for home environments.
• Provide patient and family education tailored to age, condition, culture, and learning readiness; promote infection control and universal precautions.
• Coordinate with and, when applicable, supervise other team members to ensure care aligns with patient needs and preferences.

Requirements

• Active Physical Therapist license in the state of Washington (or eligibility as required) – Required
• Current CPR certification – Required
• Valid driver’s license with reliable transportation and adequate auto insurance – Required
• 1–2 years of PT experience in acute care or rehabilitation settings – Required
• Bachelor of Science in Physical Therapy from an American Physical Therapy Association–approved program – Required
• Ability to work in a home healthcare setting and travel locally as needed
