Responsibilities

• Deliver PT evaluation, treatment planning, and ongoing re-evaluation for patients receiving in-home physical therapy.
• Implement evidence-based interventions (therapeutic exercise, gait and balance training, functional mobility training, manual therapy as appropriate).
• Develop and adjust individualized home exercise programs; provide patient and caregiver instruction for safe, independent exercise and activity modification.
• Document assessments, treatment plans, progress notes, and outcomes in the patient chart; communicate with the interdisciplinary care team to coordinate care.
• Monitor safety, infection control, and device/assistive technology use in the patient’s home; ensure compliance with practice standards and regulatory requirements.
• Manage caseload within the assigned geographic area; schedule visits to meet patient goals while maintaining quality of care.

Requirements

• Active physical therapy license in the applicable state (or eligibility to practice in that state).
• Relevant experience preferred, with ability to work in a home-based care setting.
