Job summary

Centra Healthcare Allied is seeking a travel Physical Therapist for a travel assignment in Hampton, Virginia. This role is a 13-week contract with 40 hours per week, days shift, in a SNF/LTC setting.

Requirements include graduation from an accredited program, current or pending PT licensure in the state, and CPR certification. You will develop care plans and provide skilled PT services aligned with physician orders while collaborating with the care team.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist (PT) or in the process of licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services in accordance with physician orders.
• Assess patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
