Job summary

Centra Healthcare Allied is seeking a Physical Therapist (PT) to join a SNF / LTC team for a 13-week travel assignment in the United States. Start ASAP, 40 hours per week, with 8-hour shifts.

The ideal candidate may be experienced or newly graduated, with an accredited degree, state licensure or licensure in process, CPR certification, and ability to develop plans of care and provide skilled PT services per physician orders.

Qualifications

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or in the process of licensure.
• Current CPR certification.

Responsibilities

• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions per physician orders.
• Assess patient needs, plan, evaluate and modify care to meet goals.
• Collaborate with all disciplines to plan and evaluate team goals.

A great SNF / LTC is looking for an experienced or newly graduated Physical Therapist / PT to join their growing team for a contract assignment!

• This is a contract position.
• Days schedule.
• This role assumes responsibility and accountability for incorporating the vision, values, mission and critical goals of the organization into their job performance.

Requirements include:

• Must have graduated from an accredited school.
• Current state license as a Registered Physical Therapist / PT or proactively in the process of the application process for current state licensure as a Registered Physical Therapist / PT.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assesses patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
