Centra Healthcare Allied is seeking a travel Physical Therapist for a 13-week assignment in Hampton, Virginia. The role requires a PT license, CPR certification, and the ability to plan and provide skilled PT services per physician orders.

You will assess patient needs, develop care plans, modify treatments as needed, and collaborate with a multidisciplinary team to achieve optimal outcomes across SNF/LTC settings.

Qualifications

• Graduated from an accredited school.
• Current state license as a Registered Physical Therapist or in the process of licensure.
• Current CPR certification.
• Develop a plan of care for each physical therapy patient.
• Provide skilled physical therapy services / interventions in accordance with physician orders.
• Assess patient needs, plans for, evaluates and modifies care to meet goals of physical therapy interventions.
• Collaborates with all disciplines to plan and evaluate team goals for each patient.
